Add Multiple Choice Questions
Add a multiple-choice question to the form.
When to use
We add a multiple-choice question whenever we want the customer to be able to select their answer from a list of choices.
Instructions
Add a Multiple Choice Question
- Open the survey.
- Click the Choice button.
- Type the question.
- In the Option 1 field, type the first option.
- In the Option 2 field, type the second option.
- Click the Add option button to add more.
To include 'Other' as an option, click Add "Other" option.
Reorder answers
We can reorder our answers by clicking on the 6 dots to the left of any of the options and dragging to a new location.
Multiple answers
Toggle on the Multiple answers slider to allow people to select more than one answer.
Required
Toggle on the Required slider if this question must be completed before moving on to the next question in the survey.
More options
- Click the three dots next to the Required slider to reveal more options.
Shuffle Options: This will shuffle the answers each time the form is loaded so they appear in a different, random order each time.
Drop-down: This will display the options in drop-down menus instead of as a radio button.
Subtitle: We can add a subtitle to the question to provide more context.
Add branching: We can determine the journey through the questions based on previous answers. We will be looking at branching in more detail later on in the course.
Question options
Above the question, there is a series of icons that allow us to organize and manage the questions in our survey.
These buttons allow us to copy the question, delete the question, or move the question up and down.
Preview
- Click the Preview button to see how the question looks to the user.

- Microsoft Forms incorporates AI elements so we will see a list of recommended questions we could add to the form.
- 00:04 In the previous lesson, we started to create a customer satisfaction survey,
- 00:09 and it's now time to start adding some questions into that survey.
- 00:13 And there are lots of different question types that we can add into a form or
- 00:18 survey.
- 00:19 And we're going to start out in this lesson by looking at multiple choice
- 00:24 questions.
- 00:25 Now, notice, just underneath the header area here, we have some question
- 00:30 type options, and the ones that we can see ore Choice, Text, Rating, and Date.
- 00:35 But also note that we do have a drop down arrow that contains more question types.
- 00:40 Ranking, Likert, Upload File, and Net Promoter Score.
- 00:46 Also note that we have a section option in here as well, which will basically
- 00:51 allow us to section up our survey and group together questions of similar type.
- 00:56 Now, we are going to add a multiple choice question.
- 00:59 So we want to make sure that we have Choice selected.
- 01:02 So now we get this new little pane where we can add in our question.
- 01:07 Now one thing to note here is a little bit of AI at work.
- 01:11 Because if we scroll down, you can see that we have some recommended questions.
- 01:16 So it's basically looked at the title of this survey, it seen that it's a customer
- 01:21 satisfaction survey, and it's presenting me with some common questions.
- 01:25 So I could choose any of these to quickly add a question to my survey.
- 01:30 Now we're not going to do that at this stage.
- 01:32 Let's scroll up and let's add our own question.
- 01:35 How did you hear about Happy Flame?
- 01:38 We can then add in our options.
- 01:41 So Option 1, Social Media, Option 2, from our website, and
- 01:45 then we can add more options by clicking the Add option button underneath.
- 01:50 So Option 3 is going to be from a friend, and
- 01:55 our final option will be via a Promotional Ad.
- 02:00 It's also worth noting that you can add an Other option to any of your questions.
- 02:05 So let's add that.
- 02:07 And what this will be is basically a free text field where they can type in
- 02:11 their answer if none of these other options are relevant.
- 02:14 Now another thing we can do here is we can reorder our answers.
- 02:19 So if I have my mouse over Promotional Ad,
- 02:22 notice I get this series of six dots, and that's just going to
- 02:27 allow me to pick this item up and then I can drag it to reorder it.
- 02:32 So let's just pop it into there.
- 02:34 Notice that we can add an image for each of our options as well.
- 02:38 I'm not going to, sometimes I feel that's a bit overkill,
- 02:42 but just know that you can do that.
- 02:44 And if you decide that you want to delete an entire answer,
- 02:47 you have a trash can icon next to each.
- 02:50 Final couple of things to point out here, if we scroll down,
- 02:53 we have some toggle sliders.
- 02:55 Now if I want people to be able to enter multiple answers, I could toggle this on.
- 03:01 So it means that they could select Social Media and Website, or
- 03:06 Promotional Ad and from a friend.
- 03:08 If you want to limit them to just one answer, and that's what I'm going to do in
- 03:12 this particular case, make sure you have this toggled off.
- 03:16 And it's worth noting that toggled off is the default.
- 03:19 And the other slider is if this question is required.
- 03:23 So if I were to toggle this on, it means that they're not going to be able to move
- 03:27 on to the next question in the form until they've provided an answer for this one.
- 03:32 Now, I'm not going to do that, I'm going to leave that toggled off as well.
- 03:35 And then finally, if we click the three dots to expose more options,
- 03:39 we have a few other different things that we can do in here.
- 03:43 Now we could choose to shuffle options.
- 03:45 So what this means is that when somebody loads the form,
- 03:48 it's going to display these options in a random order each time.
- 03:52 Now I'm not going to do that because I kind of want Other to always be at
- 03:56 the bottom.
- 03:56 I could choose to display these options in drop down menus as opposed to
- 04:01 radio buttons.
- 04:02 I could add a subtitle.
- 04:05 So if I click Subtitle,
- 04:06 I'm going to get this little free text field just here where I could possibly add
- 04:11 a little bit more information with regards to the context of these answers.
- 04:15 Now I'm not going to do that, so let's just turn that off.
- 04:18 And then finally at the bottom, we can add branching.
- 04:21 I'm going to speak about branching a lot more a bit later on in this course.
- 04:25 For the time being, that is our first question.
- 04:29 Also notice at the top here, we can choose to copy this question.
- 04:33 So if I wanted my next question to be reasonably similar to this,
- 04:36 I could choose to copy the question, sometimes that's a little bit quicker.
- 04:39 I can delete the entire question if I want to start again, and then I have up and
- 04:43 down arrows,
- 04:44 which is going to allow me to reorder all of the questions in the survey.
- 04:48 Now these are greyed out at the moment because currently I only have one
- 04:52 question.
- 04:52 Let's give it a little preview, I think that's looking pretty good.
- 04:56 Notice the other field, which allows for free typing.
- 04:59 Let's take a look at it on mobile, I think that looks pretty good as well.
